Admission To Health Careers
Complete Application Packet & Application Instructions
A complete application must include the following:
- ICC Application [pdf]
- High School Transcripts or GED
- Program Specific Checklists
- (Any Non-ICC) College Transcript (if applicable)
- Transcript Evaluation Form (if applicable)
- ICC College Transcript (if applicable)
- Proof of Previous Degree or Certificate (if applicable)
- Work/Volunteer Form (if applicable)
Applications for Health Career Programs are continuously being accepted. It is recommended that applications be submitted as early as possible, but not until complete with all necessary information/documentation. Review each program for its specific requirements for admission. If you do not meet program admission requirements, then you are not eligible and should not send an application for that program.
Application deadlines are dependent upon program start dates. SUMMER and FALL program application deadlin is January 15. SPRING program application deadline is October 1. Applications received after these dates will be considered only if spaces are available. Any applicant who is not accepted must re-apply for future consideration.

January 15:
SUMMER - Physical Therapist Assistant
FALL - Clinical Laboratory Technician, Dental Hygienist, Medical Assistant, Medical Coder, Medical Office Assistant, Medical Transcripptionist, Occupational Therapy Assistant, Phlebotomist, Practical Nurse, Radiographer, Registered Nurse, Respiratory Therapist, and Surgical Thechnologist.
October 1:
SPRING - Massage Therapist and Phlebotomist
Two programs, EMT (Basic and Intermediate) and Nurse Assistant, begin each semester and have spcific, individualized application processes listed.
Your completed application packet should be submitted directly to the Health Careers Department containing the infomation listed above. Below, is more specific information concerning each portion of the application.

Only paper copies are accepted. DO NOT complete an online application.
High School Transcripts
Contact your high school to obtain TWO copies of your academic transcript, each enclosed in a separate envelope. One copy must remain official. Keep it in its orginial sealed envelope and submit it in your application packet. The second copy is for your personal records and is for you to bring with you to your advisement appointments.
Applicants who are currently attending high school may apply upon completion of their junior year. High school applicants MUST call (309) 999-4600 to schedule an apointment with a Health Career advisor and bring their six-semester transcript to the appointment.
Applicants who have take the GED must request a transcript of their results (listing scores) from the Superintendent of the Educational Service Region in the area in which they reside.
Program Specific Checklists (up to 3)
For each program that you select, a separate program specific checklist is required and must be submitted in your application packet. Program checklists may be obtained online or by pick up, in person, at the Thomas Building, Downtown Peoria. Forms will not be mailed. Applicants may apply for up to three Health Career Programs; please make sure that you have met all eligibility requirements prior to submitting the application packet. Checking the box indicates that you have met that stated entrance criteria.
College Transcripts
A cumulative GPA from ALL institutions/colleges/universities from which coursework is transferred to fultill program requirements will be used to attain an all college GPA. If you intend to transfer any coursework from other colleges or universities, request TWO copies of your academic transcript, each enclosed in a separate envelope. One copy must remain official. Keep it in its original sealed envelope and submit it in your application packet. The second copy is for your personal records and is for you to bring with you to your advisement appointments.
Transcript Evaluation Form
This form is required for applicants who have coursework transferred from any institution/college/university to fultill program requirements/transfer credit. Complete and submit this form in the application packet, only if you have attended another college or university and intend to transfer any coursework.
ICC College Transcript
If you are currently attending ICC or have attended ICC after 1982, you should download and unofficial transcript through eServices (contact the ICC Help Desk at (309) 694-5457, if you need assistance) and include this in your application packet. If you attended ICC prior to 1982, please contact the Student Service Center at the East Peoria Campust, (309) 694-5610, to obtain your records.
Proof of Previous Degree or Certificate
If you have successfully completed a formal health-related degree or certificate program, submit a copyy of your documentation in your application packet.
Work and/or Volunteer Forms
If you have work or volunteer experience in a health-related field, submit the work/volunteer form. A completed work and/or voluntter form must be verified and signed by a company representative, manager, or employer and submitted in a sealed company envelope.
Application Assistance Sessions
Applicants are encouraged to attend a Health Careers Information Session for information on programs and application assistance. These sessions are held twice a month: on the second Thursday at 7:00pm and the fourth Thursday at 10:00am in the Thomas Building, Downtown Peoria, Room 109 (except holidays or when the college is closed). Additional Information
Essential Skills: It is expected that students entering the program will be able to meet the essential skills and abilities of the program with or without reasonable accommodation. Please review these skills and abilities before applying; they are listed on the specific program weeb pages.
Criminal Background: While it will have no effect on program admission, please be advised that conviction of certain crimes may affect your ability to complete the program and become licensed or certified to practice in certain states, including Illinois. See Informed Consent/Release for Criminal Background Investigation Form and note that this form does NOT need to be submitted with your application.
Admission Groups:
Preference is givent to District 514 residents folloed by residents within the State of Illinois. Some programs reserve seats according to cooperative agreements with surrounding community colleges.
Repeating Courses:
Repeating courses for improvement of application standing may negatively impact future financial aid eligibility.
