High school counselors can fill out this form to approve one or more high school students to take an ICC class.

Change of Student Information Form:  [pdf]
Students who have a change of address, phone number, or name should submit this form.

Chargeback and Cooperative Agreement Form:  [pdf]
Students who wish to attend a community college outside of the ICC community college district should fill out the form below and return it to the ICC Admissions and Records office at least 30 days before the start of the semester. For some students, there may be the possibility of attending the other community college at the in-district tuition rate.

Residency Form:
Students who recently moved to the ICC college district should read the information sheet and fill out the Residency Form below.

ICC Transcript Request Form:  [pdf]
ICC students use this form to have an official copy of the ICC transcript sent to another institution.

Request an ICC Transcript Online through Campus Direct:
Through this online service, ICC students can have official copies of their ICC transcript sent to other institutions. They can also pay for the transcripts online. This is faster than requesting a transcript by mail.
