Explanation
Exercise Guidelines
Exercise must consist of 30 to 45 minutes in a 24-hour period of time. All workout logs and printouts must be submitted to Health Services, Room 338C, by January 4, 2013.
Your home workout log should contain the date, type of exercise, and duration of activity. One hundred 30- to 45-minute workout sessions earns 75 Wellness Points. Home Exercise Log (2012) may be downloaded here. Logs are also available in Health Services, Room 338C.
If utilizing the CougarPlex, you are eligible for a Wellness Works rebate of up to $150 when you log 100, 45-minute workout sessions.
If utilizing an off campus health club, you will need to provide a printout of 100, 45-minute workout session for 150 wellness points.
Health Maintenance Exams
To receive points for completing health maintenance exams (i.e., physical, dental, vision, mammogram, PSA/digital):
- You MUST turn in proof (just marking the Wellness log will not be accepted).
- Proof can be an Explanation of Benefits Form (EOB) from an insurance company or a receipt/billing from the provider of the exam. Please DO NOT send results of the exams to serve as proof.
Dates of service for the exams must be between January 1, 2012 and December 31, 2012. As always, any health-related documentation of any form will be kept strictly confidential in Health Services. Health maintenance documentation can be submitted to Health Services anytime throughout the year, but before January 4, 2013.
Smoking Cessation
Kick the Habit refers to remaining smoke free for at least six months. Participants must already be smoke-free for six months when turning in proof. Proof is required in the form of signed letters from two co-workers and two family members verifying date of smoking cessation and continued non-smoking status. Kick the Habit may only be claimed once in a lifetime of any individual.
Weight Management
Weight Management refers to documented weight loss or healthy weight maintenance during the year. Participants must have a documented starting and ending weight for weight loss and a documented starting and ending BMI for healthy weight maintenance. Both weigh-ins/BMI must be documented from a reputable program (i.e., Weight Watchers, Jenny Craig, etc.), from a physician, or from the ICC Health Services office, or you can use your weigh in/BMI documentation from Optimum Health Solutions. The starting weight/BMI may be documented anytime but the final weigh-in/BMI must occur by December 31, 2012. The 10% and 20% amounts are calculated off the documented starting weight.
