Home Workout/Off-campus Fitness Center
A Home Workout must consist of at least 30 minutes of aerobic exercise. The following activities are examples of aerobic exercise: walking, jogging, cycling, treadmill, swimming, strength training, stair stepper and elliptical exercise machine.

You must also log your home workouts including date, type of exercise and duration of activity. Home workout logs must be turned in as proof to receive credit for points. These logs can be turned in upon completion or at the end of each session. (First Session ends June 7th / Second Session ends Nov. 1st )

Similarly, if you utilize a fitness facility (including the ICC Fitness Center) for workouts, you must log your activities including date, activity and duration to serve as proof. These logs also can be turned in upon completion and all logs MUST be turned in by the end of each session. (First Session ends June 7th / Second Session ends Nov. 1st)

Health Maintenance Exams
To receive the points for completing these exams, you MUST turn in some form of proof and the exam must have taken place between November 3, 2007, and November 1, 2008. Keep this in mind when scheduling your appointments. (If exams take place after November 1, 2008, they can be used for Wellness Works 2009) Proof can be an Explanation of Benefits Form (EOB) from the insurance company or a receipt from the provider of the exam. Please DO NOT send results of the exams to serve as proof. Proof for these exams can be turned in to Health Services, 338C, on the East Peoria Campus, as you complete it. As always, any health-related documentation of any form will be kept strictly confidential in Health Services. All proof must be turned to Health Services by the designated date at the end of the year. If the exam was completed at ICC, proof is not needed and you will automatically receive credit.

** Please Note **
Bloodwork refers to complete blood testing done for the purpose of wellness and includes a blood count, a lipid profile and a chemistry panel.

Smoking Cessation
Kick the Habit refers to quitting smoking, remaining smoke free for at least 6 months and now being a non-smoker. Participants must have quit within the last 6 months of 2007 or early 2008, as they must already be smoke-free for 6 months when turning in proof. Proof is required in the form of signed letters from 2 co-workers and 2 family members verifying date of smoking cessation and continued non-smoking status. Kick the Habit may only be claimed once in a lifetime of any individual. If smokers quit before June of 2007 and are now beyond their 6 months of being smoke free, you may claim the points for being a non-smoker. You may NOT claim both.

Weight Loss
"Dump the Plump" refers to documented weight loss during 2008. Participants must have a documented starting and ending weight. Both weigh-ins must be documented from a reputable weight loss program, from a physician, or from the ICC Health Services office. The starting weight may be documented anytime but the final weigh-in must occur between October 22 and November 1, 2008. The 10% and 20% amounts are calculated off the documented starting weight.

** Please Note **
For participants who weighed in for "Dump the Plump" 2007 but did not achieve their goal, you may use your documented starting weight from 2007 if it was done at ICC Health Services. Just let Health Services know you want to Dump the Plump in 2008 and we'll retrieve that 2007 starting weight.
