Effective April 1, 2011, the Illinois Central College Health Care Plan Description is amended so that the Mental Health and Substance Abuse Covered Providers shall include the designations shown below to be acceptable under the plan for outpatient psychotherapy under the direction of a psychiatrist or psychologist.

BCBA — Board Certified Behavior Analyst; SLP — Speech Language Pathologist; OT — Occupational Therapist

Clarification Amendment
The Illinois Central College Health Care Plan Description is amended effective for expenses incurred on or after 4/1/11 to provide that services by a Dentist are covered when related to sleep apnea diagnosis and creation of a Mandibular advancement device (MAD), or Tongue retraining device (TRD). These dental appliances will be covered at the 50% level up to the $1,200 limit.

Effective January 1, 2011, the Illinois Central College Health Care Plan Description is amended to increase the wellness benefit for Routine Mammogram and Reading to $204.00, payable once per calendar year.

Effective April 15, 2009, as an addendum to the Methodist PPO, the College began receiving a 20 percent discount on all billed charges from Methodist Medical Group and a 25 percent discount on all billed charges from Methodist Anesthesia Services.
